The privacy and protection of our patients’ personal information is very important. As a provider
of top-quality medications, the collection, use and safeguarding of this information is fundamental to our
day-to-day business operations.

What is "personal information"?
"Personal information" is specific information that enables us to identify each patient individually. It
includes but is not limited to: name and address, medical history, preferred method of payment and
medication preferences. Personal information will only be collected to the extent that is reasonably necessary to serve a legitimate business purpose and to provide the highest level of
service.

Collecting, using and disclosing personal information
We collect, use and disclose personal information for one reason: to serve you better. Knowing you better
helps us understand your medical needs, communicate effectively with you and provide you with the
services and products you want. We require your personal information to:
- Verify your identity
- Determine your eligibility for pharmaceutical products
- Comply with the provincial and national legislation that governs how we conduct business
Personal information may be collected, used or disclosed for any of the identified purposes set out above.
If it is not needed for one of these purposes, we will not use or disclose it without obtaining additional
consent from you. Patients will be notified as to how we intend to use their personal information
at the time it is collected from them.

How do we collect and store your personal information?
You provide us your personal information when you complete our Registration Form by phone, fax, mail
or internet. Completing and signing the form also provides us with your consent to use your personal
information as per above.
Most of the data we store is stored digitally. Most of the physical forms we receive, gets also digitalized & the paperwork gets shredded using a professional shredding service. The physical data we end up storing is stored in our alarmed facility & only our authorized personnel have access to it as needed to complete your order and refer to it when required. The physical prescription record will be kept by the fulfillment pharmacy as required by its jurisdiction. The digital data is stored on database servers using a dual-pass, 128-bit data encryption algorithm. Our servers are only accessible to authorized users.

How long do we keep your personal information?
Your personal information is retained only as long as we need it to effectively provide products and
services to you and as required by the fulfillment pharmacy's laws & regulations in its jurisdiction.

Access to your information
You can access your personal information stored with us at any time. Patients can examine, review and
revise their personal information by logging on to their internet accounts or by simply placing
a call to our Patient Care Department with any requests.

Keeping your information accurate
We strive to ensure that the personal information we have on file about you is as accurate and
up-to-date as necessary to be used for the identified purposes. If any information needs to be updated or
amended to reflect a change in your situation, we make every effort to update our records. We will amend
personal information that is materially inaccurate or incomplete and, where appropriate, will advise other
parties having access to the information of those amendments.
